I think you are on to something. Shorten the other boxes and move spotting and LOS to the far right, where you could type in the notes there. It would create a larger chart than the original, but everything would be in front of you without the need to flip to notes. Looking at it again I do like the format, other than the see notes. It would look to be maybe two pages?
